The biggest problems that occur during projects are caused by a lack of clarity and focus both, between different parties to the project; and internally within individual organisations.
Problems occur where activities are duplicated due to overlapping responsibilities, whilst often, at the same time other critical activities 'fall through the gap' because no one party has clear responsibility.
